Introduction

The department of Burns & Plastic Surgery is committed in providing comprehensive patient care with utmost dedication and catering to all the underprivileged patients presenting from near and far. The department in addition to its extensive routine surgeries works in collaboration with various other departments such as Trauma & Emergency, neurosurgery, cardiac surgery, orthopedic surgery, ENT, dentistry etc. The ambitious goal of this collaborative team of zealous and enthusiastic team of surgeons is to reduce the morbidity and provide functionality by reconstructive procedures including advanced microsurgical techniques thereby providing an improved quality of life to the patients. It caters to almost all emergency hand injuries, microvascular reconstruction for complex limb trauma, post burn reconstruction, maxillofacial trauma, oncoplastic reconstruction, oncoplastic breast reconstruction and other congenital anomalies like cleft lip, cleft palate, congenital hand deformities (syndactyly, polydactyly, camptodactyly, hypoplastic thumb deformity, etc.) advanced wound management using Negative Pressure Wound Therapy system (NPWT) and even providing excellence in aesthetic care including aesthetic breast surgery, aesthetic facial surgery and other procedures like gynecomastia, abdominoplasty, brachioplasty, thighplasty and total body contouring procedures, etc. The department is recognized all over the state for its pioneering work in microvascular and brachial plexus reconstructive surgeries.

The department has four Faculty members, 1 Additional professor, 2 Associate professors and 1 Assistant Professor. The faculty members are supported by 6 M.Ch. senior resident, 2 junior residents, 1 Staff nurse, 1 Lower division Clerk, 1 OT technician and 3 Hospital attendants. The M.Ch (Plastic & Reconstructive Surgery) program has been started from January 2021 session with an annual intake of three M.Ch. residents. Currently we have 6 senior resident in our M.Ch program.

The department has started the operation of its LASER Suite which encompasses 2 well equipped cubicle harboring a highly advanced LASER Machine equipped to perform a wide range of restorative as well as aesthetic procedures including Vascular lesions, pigmentary lesions, chronic scarring, hair removal and facial rejuvenation.

At present, a state of the art Burns Centre is in the midst of construction. This center will cater to burn patients from all over Bihar, Jharkhand and other eastern states of the country with facilities for ICU, skin bank and regenerative medicine laboratory.

Highlights for the year:

  1. The department vigorously participated in the AIIMS declaration as a Dedicated COVID Hospital (DCH) with its wholehearted contribution in the exemplary care of COVID patients.
  2. Started its LASER services both as restorative and aesthetic procedures with a state-of-the-art machine available only in few government centers.
  3. Continuing its liposuction services catering to the unprivileged patients not able to afford standard care elsewhere.
  4. Regularly using Venous Couplers for venous anastomosis in microvascular free flaps, a latest technology available only at 4-5 centers in India.
  5. Faculty members participated as Resource persons for Plastic Surgery Team in surgical camps organized by Lifeline Express at Sitamarhi (district of Bihar) under Community Outreach Program of the Institute.
  6. Faculty members participated as Resource persons in a dedicated team for actively conducting Critical Thinking Foundation Course (CTFC) Program of the Institute.
  7. Faculty members participated during “Breast cancer awareness week” and conducted public awareness on “Breast reconstruction” to the family members of institute staff.
  8. Faculty members are associated with regular online webinars conducted by the Specialties’ National Association of Burns India (NABI) for conduction of extensive awareness campaigns as well as Post graduate teaching.
  9. A faculty member served as a resource person for conducting health camps and raising awareness during National Road Safety Week and National Fire Safety Week, organised by the Bihar Government's Department of Disaster Management.

One of the major highlights of the department in the year 2019-20 has been offering free of cost surgery for patients with cleft lip and palate under Rashtriya Bal Swasthya Karyakram (RBSK), a national project under Govt. of India. The department faculty is the Nodal Officer for the same and coordinates with the other departments like Paediatric Surgery, ENT, Orthopedics, Paediatrics, Opthalmology, and Dentistry for the surgical management of congenital deformities covered under this program.

The department and OPD is located at 5th floor, OPD block. In ward, the current bed strength is 30 in D5B block, IPD building. There is one dedicated operation theatre in central O.T. complex 5 days/week.

Clinical

Although the State being affected by the severe COVID Pandemic the department in association with the institute continued to offer its exceptional services in clinical care to all its patients.

The department is providing Outpatient care from Monday to Saturday dealing in all the aspects of plastic surgery. Specialty clinics of keloid, vascular malformation, hand clinic and cleft clinics are run in the second half on a weekly basis. The department has catered to around 1200 patients in the previous year, the tally being affected by the COVID pandemic

The department has a 30 bedded plastic surgery inpatient ward with dedicated staff for the care of the admitted patients. Total admissions in the last year were 99.

Surgeries are being done daily from Monday to Friday in the newly opened OT complex with 1 OT dedicated to Burns & plastic surgery. Apart from providing elective surgeries, emergency surgeries are also being performed on a need to basis in the trauma OT complex. The department continued specialized surgeries like microvascular reconstructions, brachial plexus surgeries, trauma, hand surgeries, cleft lip & palate surgeries etc. The department has performed over 84major surgeries in the last 3 months of last year. Although the department could not perform to its full potential due to closure between 01/04/2021 to 31/12/2021, as the O.T. services were closed due to AIIMS Patna being declared a 'Dedicated Covid Hospital (DCH)'.

In the last year, newer technology like use of venous couplers for venous anastomosis in microsurgical procedures has started in the department. Additional services like Liposuction have been started for patients undergoing aesthetic procedures.

Non-clinical

The department faculty members participated as members of various committee (Oration committee, Spandan committee, Stage committee) in the Organizing team for Institute Foundation Day celebrated on virtual platform on 25th September 2021. The faculty member has also contributed as member of Editorial team, Annual report of AIIMS Patna.

Teaching and Training

The department is proactively involved in teaching and training activities of undergraduate, postgraduate as well as nursing students including lectures, clinical postings and departmental teachings. The Faculty members are also enlisted as editorial board members, experts of review committees in reputed national and international journals. All the members actively participated in training of medical and police personnel which were organized by institute in collaboration with Dept. of Disaster Management, Govt. of Bihar educating about first aid and CPR.

Future Planning

The future aim of the department is to establish the state-of-the-art Tertiary Burn center at AIIMS which could benefit the hugely deprived population of Bihar. The department also plans to start craniofacial surgeries, ICG Fluoroscopic mapping for intraoperative flap monitoring, Endoscopic plastic surgery. In the training domain, the process for establishing a microsurgical skill lab for developing microsurgical skills of residents and trainees is already in the process.
